Many Providers are losing too much money on simple mistakes! Before turning in your claim for next month do these quick checks.
ATTENDANCE SHEETS:
1.	Check menus and attendance forms BEFORE mailing or bringing into the office. Same mistakes are being made every month.
2.	Bubbles not filled in or filled in wrong. Wrong ID numbers, social security numbers, dates, etc.
3.	Too many folds cause jams in the scanner, or misreads. We provide larger envelops if you need them.
4.	Too many erasers, use whiteout( liquid paper), or get another sheet.
5.	Do not staple attendance forms to menus. Staples put holes in sheet and scanner rejects the form.
6.	Claim MUST be in the office by the THIRD of the month or it is late. It can take three to six months to get your money if late.
7.	Check children ID’s to make sure you are using the correct number. The wrong ID will cause you to lose the entire child for the month. This includes if it is bubbled in wrong. State will not pay for your mistake
8.	If you don’t have the ID number and you mailed or brought in the enrollment before the end of the month, call to get it before you send in your menus.
MENUS:
1.	Make sure you have milk in the milk section, not juice, yogurt, or water.
2.	Potatoes are not bread, do not put it in the bread section, it is a vegetable.
3.	On breakfast, lunch, supper, all lines must be filled in and have the correct item on them.
4.	For tacos, spaghetti, stews, casseroles, soups, pizzas, etc., you can only claim two of the ingredients. EX: TACO- SHELL AND MEAT OR CHEESE AND TOMATO. NOT ALL FOUR.
5.	If a component is served on the side (separate) you must write “served on the side” next to the item.
EX.- PIZZA- CHEESE AND DOUGH, “SLICED TOMATOES ON THE SIDE”
6.	Jelly is not a fruit, unless it is 100% fruit jelly. 
7.	Make sure you have a menu for every bubble you fill in on the attendance sheet.
8.	If you are going to serve the same thing each week and you make copies of that menu for the month, make sure you don’t have the same mistakes, otherwise you will get the same disallowance each week.
9.	You cannot serve orange juice and orange slices as a snack. They are the same family group. Orange juice and apple slices are fine.
10.	Water is not creditable as a food item even at snacks.
